The Bill Moore Community Health
Clinic is a State licensed Community
Clinic which provides free or low cost primary health care clinic to
low income and indigent children, adults and seniors. Services include
physical examinations, medical treatment, well-baby care, general
pediatric care, women’s health care, including mammograms and pap
smears, immunizations, TB testing, STD testing, WIC exams, health
education and case management. Referrals are made for specialty
services as appropriate. Needed ancillary services including
medications, laboratory services and radiology services are provided.
The clinic is part of the State EAPC Program, the CDP (Cancer Detection
Program, for breast and cervical cancer screening) and the Family Pact
Program. Medical services are provided by physicians and nurse
practitioners. Funding for these services is provided by the State
Department of Health Services, private foundations, Medi-Cal, private
donations and fee for services.
The
Clinic has special medical
programs for women and children
funded through the California Wellness Foundation and other Foundations.. The Clinic also has a special Chronic Disease
Management Program for persons with diabetes. These services are
partially funded by The California Endowment and are a collaborative
service with Madison Neighborhood Partners and the Pasadena Public
Health Department. Other funding for the Clinic comes from the Patron
Saints Foundation, Kaiser Permanente, The Rose Hills Foundation, Blue
Cross of California Foundation, Unihealth, Tides Foundation, Guenther
Foundation as well as individuals and groups concerned about the high
number of individuals and families who lack health insurance.
An important focus of the
Clinic is the prevention of chronic diseases and the provision of
health education to the community to this end.
